29 CFR 1910.23(c)(1): Open sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides: a) Cyclone Drilling, Inc. Rig #29 at RGU 24-25-198 in Meeker, CO 81641: On and before 7/25/12, the employer did not ensure that the mud pit was equipped with a standard railing to guard against falling in that there was no intermediate rail. This condition exposed employees to a fall hazard of 15 ft from the mud pit. b) Cyclone Drilling, Inc. Rig #29 at RGU 24-25-198 in Meeker, CO 81641: On and before 7/25/12, the employer did not ensure that the premix tank owned by Zimmerman Equipment Company was equipped with a standard railing to guard against falling in that there was no railing. This condition exposed employees to a fall hazard of 10ft from the premix tank. Rig #29 at RGU 24-25-198 in Meeker, CO 81641: On and before 7/25/12, the employer did not ensure that flights of stairs with 4 or more risers, less than 44 inches wide and having both sides open, were equipped with a standard handrail on each side. A stair with 4 risers or more provided access to the mud pits used by the active well. The handrail was missing on the left side descending. This condition exposed employees to a fall hazard. Rig #29 at RGU 24-25-198 in Meeker, CO 81641: On and before 7/25/12, the employer did not ensure that storage of material did not create a hazard. Bags of chemical inventory were stacked on top of the trailer and were not stable and secure against sliding or collapse. This condition exposed employees to a crush by hazard.
