29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. On or about July 31, 2012, the employer did not ensure employees were in use of fall protection while working from the roof of a building with unprotected sides and edges exposing the employees to a fall of more than 6 feet to the ground below.

29 CFR 1926.1051(a): A stairway or ladder was not provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided. On or about July 31, 2012, the employer did not ensure a stairway or ladder was provided for all employees working from a roof with a break in elevation more than 19 inches exposing employees to a fall hazard.
