Occupational Safety and Health Act of 1970 Section (5)(a)(1):  The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or were likely to cause death or serious physical harm to his employees in that employees were exposed to struck-by and/or crushed-by hazards:    This violation most recently occurred on August 3, 2012, at a workplace located at 5940 Forest Park Rd., Dallas, Texas 75219; where at least two (2) employees were exposed to the hazards of being struck-by and/or crushed-by the rotating superstructure of a truck-mounted drill rig.

29 CFR 1926.501(b)(7)(ii): Each employee at the edge of a well, pit, shaft, and similar excavation 6 feet (1.8 m) or more in depth was not protected from falling by guardrail systems, fences, barricades, or covers:    This violation most recently occurred on August 3, 2012, at a workplace located at 5940 Forest Park Road, Dallas, Texas 75219; where at least two (2) employees, who worked at or near the unprotected edge of 42-inch and 48-inch diameter pier holes while measuring the depth, were exposed to the hazard of falling 15-feet, 8-inches and 19-feet. 8-inches to the lower level.
