29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement: a) Courtyard by Marriott, Worksite, Excavation - On or about June 8, 2012, employees are conducting cast in place concrete construction work that requires the installation of reinforcing steel bar sections that are not properly guarded.

29 CFR 1926.1053(b)(15): Ladders were not inspected by a competent person for visible defects on a periodic basis and after an occurrence that could affect their safe use: a) Courtyard by Marriott, Worksite - On or about June 8, 2012, employees were using a job-made wooden ladder, constructed without filler blocks installed between each cleat, that was not removed from service after inspection by the competent person. ABATEMENT NOTE: Among other methods, one feasible means of abatement would be to build job-made ladders per the requirements of American National Standards Institute Standard A14.4 - 1979 which specifies the need for filler blocks in Section 4.5.2.
