29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection. a) 1200 Clifford Street, Wilmington, DE - an employee was using a flexible cord plugged into a defective outlet that was not GFCI protected. This exposed the employee to an electric shock hazard. The violation was observed on or about July 31, 2012.

29 CFR 1926.404(b)(1)(iii)(C): The employer did not ensure that each receptacle which are fixed and exposed to damage, were not visually inspected before each day's use for external defects, such as deformed or insulation damage, and for indications of possible internal damage. a) 1200 Clifford Street, Wilmington, DE - an employee was using a 120 volt electric outlet that was pulled away from the wall, had the conduit pulled apart and was not protected by a GFCI. This exposed the employee to an electric shock hazard. The violation was observed on or about July 31, 2012.
