29 CFR 1926.501(b)(1):     Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a) Roof of the one-story facility -- On or about July 16, 2012, two employees were working along the perimeter edge of the roof of a one-story non-residential building, no less than 10 feet, 11 inches above grade, without any means of fall protection.

29 CFR 1926.1053(b)(1):     Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigied support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder.    a) Roof of the one-story facility -- On or about July 16, 2012, the rails of a portable extension ladder did not extend 3 feet above the upper landing surface and was not secured at its top to a rigid support which did not allow for a stable means of access or egress at a height of 10-feet, 11 inches above grade.
