29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects. a) Jobsite: 1544 S 13th St, Philadelphia, PA. - On or about 7/31/12, an employee working without head protection was exposed to head injuries from falling objects such as demolished bricks from the face of the building. The employee was assisting with the masonry operations by hoisting materials to co-workers on scaffold from ground level.

Employees were performing masonry operations approximately 27 feet above ground level.

FRESH START BUILDERS WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD, WHICH WAS CONTAINED IN OSHA INSPECTION# 314235813, CITATION# 1, ITEM# 1A, AND WAS AFFIRMED AS A FINAL ORDER ON 9/27/11, WITH RESPECT TO A WORKPLACE LOCATED AT 2232 SOUTH BROAD STREET, PHILADELPHIA, PA.
