(b) Jobsite, Garage, 928 4th Street, East McKeesport, PA: On or about 08/01/12 employees engaged in residential construction activities approximately 9 feet above the ground were not protected from falling by the means of guardrail systems, safety net systems or personal fall arrest systems.

29 CFR 1926.503(c): "Retraining." When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer shall retrain each such employee: (a) Jobsite, 928 4th Street, East McKeesport, PA: On or about 8/1/12 retraining was not conducted for employees, who had been trained, but did not retain the requisite understanding and skill to protect themselves from fall hazards. Employees were working at heights between 9 and 24 feet without utilizing any means of fall protection.
