In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

NOTE: On roofs less than 50 feet wide safety monitoring alone can be used:    a) Employees were exposed to the hazard of falling approximately 16 feet while performing roofing activities without the use of fall protection or a safety monitoring system.    Pro-Tech Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501 (b)(10) which was contained in OSHA inspection number 311374136, citation 1, item 1, issued on May 29, 2008, and became final order on or about June 17, 2008.
