29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about 08/03/2012, the employer failed to ensure that he and his workers were protected from falling approximately 9 to 19 feet from a steep roof (6 in 12 pitch) while performing roofing work.

29 CFR 1926.1053(b)(1): Where the side rails of a portable ladder used for access to an upper level/landing surface did not extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that would not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder: On or about 08/03/2012, the employer failed to ensure that he and his workers had means to safely mount and dismount the top of a ladder used to access the lower ledge of the roof from the ground. Workers performing roofing activites used a Werner extension ladder leaned against the building to access a 9 foot tall roof ledge; the side rails barely extended over the roof/landing.
