Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws.    On or about August 3, 2012, the employer failed to ensure that flexible cords had adequate strain relief. The flexible cord was used by employee(s) as a power source for a shop fan.
