29 CFR 1910.119(e)(7): Employers shall retain process hazards analyses and updates or revalidations for each process covered by this section, as well as the documented resolution of recommendations described in paragraph (e)(5) of this section for the life of the process. The employer did not ensure that process hazard analyses for the ammonia system installed prior to 1997 was retained and updated. The initial process hazard analysis was requested by OSHA on August 9, 2012 and January 7, 2013 but was not received by OSHA. The employer only provided the 2008 PHA based on modification and expansion of the ammonia system.
