29 CFR 1926.501(b)(11):     Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:      a) West End/Roof of the Multi-story facility -- On or about July 17, 2012, employees engaged in roofing work on a steep roof of a multi-story, non-residential campus office building were not protected by guardrail systems, safety net system, or personal fall arrest system, exposing employees to a fall hazard of no less than 7 and up to 25 feet.

29 CFR 1926.1053(b)(1):     Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a) Along the facilitys west wall - On or about July 17, 2012, the rails of a portable extension ladder used for access to the roof did not extend 3 feet above the upper landing surface which did not provide a stable area of transition for access and egress no less than 18 feet above grade.
