29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: a) Jade Group Construction at 9016 - 9018 Sandpiper Drive, Frederick, CO 80504: On or before August 4, 2012, the employer did not ensure that employees wore safety glasses when using or working in the vicinity of pneumatic nail guns. Condition exposed employees to the hazard of projectiles to the eyes.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: a) Jade Group Construction at 9016 - 9018 Sandpiper Drive, Frederick, CO 557778: On or before August 4, 2012, the employer was using a yellow extension cord to power a Milwaukee Sawzall, an orange extension cord to power a Milwaukee drill, and a blue extension cord to power a Roll-Air air compressor. Each cord had an open ground. Condition exposed employees to electrical shock hazards.
