29 CFR 1926.1053(b)(1):     When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.    Employees using a portable Step ladder to access a roof were exposed to fall hazards.  The ladder side rails extended approximately two foot above the upper landing surface.

Employees using an aluminum step ladder to access a roof were exposed to fall hazards up to 6 feet. The employees had  not recognized hazards related to proper design and use of ladders and procedures to follow in order to minimize those hazards.

The program shall enable each employee to recognize hazards related to ladders and stairways, and shall train each employee in the procedures to be followed to minimize these hazards.    Employees using an aluminum step ladder to access a roof were exposed to fall hazards of up to 6 feet.  The employees had not been trained so far as to enable each employee to recognize hazards related to ladders and procedures to follow in order to minimize those hazards.

29 CFR 1926.20(b)(1):     It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part.    The employer had employees exposed to fall hazards, on a residential construction site in Pekin, Illinois.  The employer did not have an effective safety and health program with specific work rules, competent person oversight, and employee training to govern employees activities.

29 CFR 1926.20(b)(2):     Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.    Employees conducting residential roofing activities were exposed to fall hazards by improper use of a ladder.  The jobsite was not inspected by a competent person.
