29 CFR 1926.100(a):  Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a.  On or about July 24, 2012, at the southwest of the building located at 907 Taylor Ave, Port Orange, FL, an employee working for Central Florida Construction Walls, Inc., was not protected by a hardhat while hammering a lath metal strap above his head, exposing himself to head injuries.

29 CFR 1926.451(e)(1):  A means of access for scaffold platforms more than 2 feet (0.6 m) above or below a point of access, such as portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used.     a.  On or about July 24, 2012, at the southwest of the building located at 907 Taylor Ave, Port Orange, FL, employees working for Central Florida Construction Walls, Inc., were climbing the frame of the scaffold to gain access to the roof, exposing themselves to an 8.3 feet fall hazard.     Central Florida Construction Walls, Inc., was previously cited for a violation of this safety and health standard or its equivalent standard 29 CFR 1926.451(e)(1) which was contained in inspection number 316517914, citation 1, item 3, issued on March 28, 2012, with respect to a work place located at 11731 Colonial Dr., Orlando, FL 32826.  The final order date was May 17, 2012.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and fuardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.      a.  On or about July 24, 2012, at the southwest of the building located at 907 Taylor Ave, Port Orange, FL, employees working for Central Florida Construction Walls, Inc., were doing stucco work on a 4:12 pitch roof and not being protected by a guardrail system, safety net or personal fall arrest system, exposing themselves to an 8.3 feet fall hazard.     Central Florida Construction Walls, Inc., was previously cited for a violation of this safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10) which was contained in inspection number 316517914, citation 1, item 6, issued on March 28, 2012, with respect to a work place located at 11731 Colonial Dr., Orlando, FL 32826.  The final order date was May 17, 2012.

On or about July 24, 2012, at the southwest of the building located at 907 Taylor Ave, Port Orange, FL, employees working for Central Florida Construction Walls, Inc., assembled a tubular frame scaffold without been trained by a competent person in the correct procedures for erecting, disassembling, operating, repairing, inspecting, and maintaining, exposing themselves to fall hazards.
