On or about July 24, 2012, at the southwest of the building located at 907 Taylor Ave, Port Orange, FL, employees working for Cold Formed Systems, LLC, were carrying and cutting wood on a 4:12 pitch roof and not being protected by a guardrail system, safety net or personal fall arrest system, exposing themselves to an 8.3 feet fall hazard.

29 CFR 1926.1053(a)(3)(i): Rungs, cleats, and steps of portable ladders and fixed ladders (including individual-rung/step ladders) were spaced less than 10 inches (25 cm) apart, or more than 14 inches (36 cm) apart, as measured between center lines of the rungs, cleats and steps: a. On or about July 24, 2012, at the northeast of the building located at 907 Taylor Ave, Port Orange, FL, the portable job-made wood ladder belonging to Cold Formed Systems, LLC, and used by employees of Air Systems of Daytona, Inc., to gain access to the roof had the rungs spaced at about 8 inches apart, exposing themselves to a fall hazard.
