29 CFR 1926.451(c)(1)(ii):     Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by installing guys, ties, and braces according to the manufacturer's recommendation:    (a) Jobsite, 123 Apple Street, Connellsville, PA:  On or about 08/02/12 the supported scaffold with a base width of 5 feet and a height of approximately 30 feet was not restrained from tipping by installing guys, ties or braces.  Employees were working on the scaffold at the highest level (30 feet) and there was no ladder for access.  The employees climbed the scaffold frames and cross-braces and this condition increased the risk of tipping from not using guys, ties or bracing.

29 CFR 1926.451(c)(2):     Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:    (a) Jobsite, 123 Apple Street, Connellsville, PA:  On or about 08/02/12 the supported scaffold pole legs on the section of the scaffold approximately 30 feet in height did not bear on base plates and mud sills or other firm foundation.  The scaffold leg had 2 pieces of 2 x 4 boards taped around it and the leg was sitting on 3 pieces of 1 inch thick boards ranging in size from 2 inches to 5 inches wide.

29 CFR 1926.451(e)(1):     When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access:    (a) Jobsite, 123 Apple Street, Connellsville, PA:  On or about 08/02/12 there were no ladders, stair towers, ramps or walkways used to access the scaffold that ranged in height from 20-30 feet.  The employees accessed the uppermost levels by climbing the cross braces and frames.

(a) Jobsite, 123 Apple Street, Connellsville, PA:  On or about 8/02/12 the employer did not train employees who work on the scaffold to recognize the hazards associated with the type of scaffold being used or the procedures to control or minimize those hazards.  Employees were working on a scaffold with the following hazards:  no fall protection, no safe access or adequate foundations.
