29 CFR 1910.132(d)(2):     The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.    a)  At Pioneer City Casting, located in Belpre, Ohio the employer had not performed a written hazardous assessment to certify which Personal Protective Equipment should be worn while performing different tasks in the workplace.

29 CFR 1910.212(a)(1):     Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    a) At Pioneer City Casting, located in Belpre, Ohio, two drill presses Manufactured by Walker - Turner Model # Unknown Serial # 52DK2A, in the Machine Shop,  had a rotating chuck and cutting bit that was not guarded exposing employees to amputation hazards.    b) At Pioneer City Casting, located in Belpre, Ohio, a drill press Manufactured by Duracraft, Model # DP-1617, Serial # 20405, in the Machine Shop,  had a rotating chuck and cutting bit that was not guarded exposing employees to amputation hazards.    c) At Pioneer City Casting, located in Belpre, Ohio, a drill press Manufactured by Atlas, Model # Unknown, Serial # Unknown, in the Machine Shop,  had a rotating chuck and cutting bit that was not guarded exposing employees to amputation hazards.    d) At Pioneer City Casting, located in Belpre, Ohio, a drill press Manufactured by Walker - Turner, Model # 1113-41, Serial # 787, in the Machine Shop,  had a rotating chuck and cutting bit that was not guarded exposing employees to amputation hazards.    e)  At Pioneer City Casting, located in Belpre, Ohio, a drill press Manufactured by Clausing, Model # 2234, Serial # 105054, in the Machine Shop,  had a rotating chuck and cutting bit that was not guarded exposing employees to amputation hazards.    f)  At Pioneer City Casting, located in Belpre, Ohio, a drill press Manufactured by Clausing, Model # Unknown, Serial # Unknown, in the Machine Shop, had a rotating chuck and cutting bit that was not guarded exposing employees to amputation hazards.    g)  At Pioneer City Casting, located in Belpre, Ohio, a drill press Manufactured by Clausing, Model # 2215, Serial # Unknown, in the Machine Shop,  had a rotating chuck and cutting bit that was not guarded exposing employees to amputation hazards.    h)  At Pioneer City Casting, located in Belpre, Ohio, a drill press Manufactured by Cincinnati - Bickford, Model # Unknown, Serial # Unknown, in the Machine Shop,  had a rotating chuck and cutting bit that was not guarded exposing employees to amputation hazards.    i)  At Pioneer City Casting, located in Belpre, Ohio, a drill press Manufactured by Fosdick, Model # Unknown, Serial # Unknown, in the Machine Shop,  had a rotating chuck and cutting bit that was not guarded exposing employees to amputation hazards.
