29 CFR 1926.652(b)(2): Maximum allowable slopes, and allowable configurations for sloping and benching systems, were not determined in accordance with the conditions and requirements set forth in appendices A and B to this subpart: a) Job site, 2723 West Liberty Road, Pittsburgh, PA: On or about July 12, 2012, during retaining wall construction activities, the employer did not ensure that appropriate cave in protection was used while an employee worked on the ledge area to perform beam layout and installation work. The wall angles on the excavated hillside above the ledge area were determined to be composed of class "b" soils, the walls approximately 10-15 feet tall in many areas above the ledge and the angle the wall above the ledge was angle to was approximately 75-80 degrees, exposing workers to wall collapses during this work.
