No. 10A-1970:    A.)  Erie Sign Corporation, 2747 West 12th Street, Erie, PA  16505, company vehicle, on or about July 25, 2012 and times prior thereto:  An Amerex B456 10# multi-purpose dry chemical fire extinguisher (S/N:  YC-329874) had not recieved an annual maintenance check.  The fire extinguisher's last annual check was in 2006.    B.)  A.)  Erie Sign Corporation, 2747 West 12th Street, Erie, PA  16505, company vehicle, on or about July 25, 2012 and times prior thereto:  A First Alert FE3A40 10# multi-purpose dry chemical fire extinguisher (S/N:  WY-957880) had not recieved an annual maintenance check.
