29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:    a.  On or about July 24, 2012, inside the building located at 907 Taylor Ave, Port Orange, FL, employees working for Simplex, Inc., were using a DeWalt Heavy Duty 12 Single Bevel Compound Miter Saw that had a damaged electrical cord, in that the strain relief was broken at the plug end and also had frayed uncovered wire leading into the saw, exposing themselves to electric hazards.
