29 CFR 1910.146(c)(5)(ii)(B): When entrance covers are removed, the opening shall be promptly guarded by a railing, temporary cover, or other temporary barrier that will prevent an accidental fall through the opening and that will protect each employee working in the space from foreign objects entering the space. On or about 08/06/2012, the employer did not ensure that when the expanded metal cover was removed from the opening to a crawl space beneath the building, barricades were not erected to prevent anyone falling into the openong. The opening measured 24"x36" and was approx. 36" deep at the opening

29 CFR 1910.146(c)(5)(ii)(C): Before an employee enters the space, the internal atmosphere shall be tested, with a calibrated direct-reading instrument, for oxygen content, for flammable gases and vapors, and for potential toxic air contaminants, in that order. Any employee who enters the space, or that employee's authorized representative, shall be provided an opportunity to observe the pre-entry testing required by this paragraph. On or about 08/06/2012, the employer did not ensure that air monitoring of the crawl space beneath the structure was conducted prior to employees entering. The crawl space beneath the structure was approx 100 ft long with two side crawl spaces that were perpendicular to it. The crawll space was 4 ft square and contained a substantial amount of standing water inside.

29 CFR 1910.146(d)(9): Develop and implement procedures for summoning rescue and emergency services, for rescuing entrants from permit spaces, for providing necessary emergency services to rescued employees, and for preventing unauthorized personnel from attempting a rescue; On or about 08/06/2012the emplpoyer did not develop procedures to rescue employees working in a confined space. Two employees were working in the confined space beneath the structure, without any means of rescue and retrieval of injured employees.

29 CFR 1926.404(a)(2): Polarity of connections. No grounded conductor shall be attached to any terminal or lead so as to reverse designated polarity. On or about 08/06/2012, the employer did not ensure that the outlets on the temporary power pole at the rear of the structure, were wired correctly. Outlets were found to have two "hot" terminals unstead of a hot and neutral. This exposed site personnel to electric shock hazards.
