Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.      a) Holiday Inn Hotel Roof, North Face - Roofers were observed working on a low slope roof at 60 feet above ground and were not protected against fall hazards, on or about 08/01/12.
