OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of falls through equipment failure:     On or about 08/08/12 pins provided for an Up-Right Inc., Model 62697-002, S/N 5747 lift had been replaced with bolts.  These replacement parts for the aerial manlift were not approved by the manufacturer. The bolts did not fit as tight as the missing pins making for an unstable work platform when the lift was raised.  The lift was used to replace light bulbs and clean lighting fixtures above the pews, located in the sanctuary.  One method of abatement among others is to follow the operating manual provided by the manufacturer for this lift:  Ref:  "NEVER replace any component or part with anything other than original Up-Right replacement parts without the manufacturers consent."    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the oblication, including describing the steps that it is taking to ensure that replacement parts for the manlift are approved by the manufacturer.
