29 CFR 1910.1052(h)(1):     Where needed to prevent methylene chloride-induced skin or eye irritation, the employer did not provide clean protective clothing and equipment which is resistant to methylene chloride, and did not ensure that each affected employee used it.   *******************    a)  United Church of Christ Jobsite:  Employees were using a methylene chloride based paint remover on the structure.  They did not use impervious gloves while applying the paint stripper.  In addition, not all the employees used eye protection to protect against splashes into the eyes.

29 CFR 1910.1052(i)(2):     Where it was reasonably foreseeable that employees eyes may contact solutions containing 0.1 percent or greater methylene chloride (for example through splashes, spills or improper work practices), the employer did not provide appropriate eyewash facilities within the immediate work area for emergency use.     ************************************      a)  United Church of Christ Jobsite:  Employees were using a methylene chloride based paint remover on the structure.  The employer did not have available in the immediate area appropriate eyewash facilities for emergency use.

The company did not have a material safety data sheet (MSDS) for that product at the jobsite.

29 CFR 1926.62(l)(1)(i):     The employer did not communicate information concerning the hazards of lead exposure according to the requirements of OSHA's Hazard Communication Standard for the construction industry, 29 CFR 1926.59, including but not limited to the requirements concerning warning signs and labels and employee information and training:  *****************************************    a)  United Church of Christ Jobsite:  During the initial day(s) of scraping the lead-containing paint off of the church clapboards, the employer did not put up the "lead hazard" warning tape and the crew had not been made aware that the existing paint on the building contained lead and was a potential health hazard if the dust was breathed or ingested.

29 CFR 1926.404(b)(1)(ii):     On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building and which are in use by employees did not have approved ground fault circuit interrupters for personal protection.   ***************************************************    a)  United Church of Christ Jobsite:  The crew did not plug their extension cord into a "ground fault circuit interrupter" protected outlet.  The extension cord supplied power to a multi-outlet electrical box, to the vacuum cleaner, and to a Makeda disc sander.  An assured equipment grounding program was not in use.
