29 CFR 1926.501(b)(13):     Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b)    a)  160 Stone Park Blvd, Pike Road, Al, building 500:  The employer failed to ensure employees working from the breezeway and walking accross it were protected from falls by having guardrails installed which exposed them to the hazards associated with fall of 9 feet 9.5 inches to the ground below.    b)  160 Stone Park Blvd, Pike Road, Al, building 500:  The employer failed to ensure an employee working from trusses over the breezeway was protected from falls by using personel fall arrest while shoring up trusses which exposed him to the hazards associated with fall of 8 feet 4.5 inches to the breezeway below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a)  Building 500, on or about August 2, 2012; The employer failed to ensure the ladder used to access the second level ceiling area extended at least 3 feet above the landing which exposed employees to the hazards associated with fall hazards of up to 9 feet 9 1/5 inch to the decked floor below.

29 CFR 1926.1053(a)(2):     Ladder rungs, cleats, and/or steps were not parallel, level, and uniformly spaced when the ladder was in position for use:      a) Building 500, on or about August 2, 2012; The employer failed to ensure a job made ladder was built or maintained with uniformed rungs throughout the ladder which exposed employees to the hazards associated with falls.
