29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) Blagburn Glass and Panel on or about August 08, 2012, at the address of 730 W. Market St. Lima, OH: The employer did not ensure employees were protected from falls while installing paneling material from a low sloped roof deck. While performing this work from the unprotected elevated surface, employees were exposed to falls of approximately 16ft to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. a) Blagburn Glass and Panel on or about August 08, 2012, at the address of 730 W. Market St. Lima, OH: The employer did not ensure that employees had been trained in the appropriate use of fall protection such as but not limited to the use of fall protection systems or alternative measures while performing paneling installation operations from elevated surfaces.
