29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least three (3) feet (.9 m) above the upper landing surface being accessed: a/ At the job site, a portable step ladder was used by employees to gain access to the attic from the fourth floor through a shuttle hole in the ceiling. The top step belonging to this ladder was approximately six inches below the ceiling. The ceiling was approximately nine feet above the floor.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards: a/ At the job site, exposed employees were not instructed to recognize a fall hazard posed by the insufficient length of a portable step ladder. These employees used this portable step ladder to gain access to the attic through a shuttle hole in the ceiling.

29 CFR 1926.252(a): An enclosed chute of wood, or equivalent material, was not used where materials were dropped more than 20 feet to points lying outside the exterior walls of the building(s): a/ At the job site, employees were dumping roofing debris from a four-story high roof and into a dumpster on the ground adjacent to the front entrance to the facility. No enclosed chute or the equivalent were in use.
