29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner:    On or about July 31, 2012 and at times prior thereto, employees were working on a mobile scaffold and the wheels were not locked to prevent movement of the scaffold while they were working on it.  Scaffold was set up on a roof section over the walkway, the height of the scaffold was approximately 5 feet and the roof top was approximately 16 feet from the ground.  Employees were exposed to a fall of at least 15 feet.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    On or about July 31, 20121 and at times prior thereto. employees were working on a roof section overhang above the walkway without fall protection.  The area of the roof section was approximately 8 feet wide by 30 feet long and approximately 16 above the ground.  Employees were exposed to a fall of over 10 feet.
