29 CFR 1926.701(b): Reinforcing steel. All protruding reinforcing steel, onto and into which employees could fall, was not guarded to eliminate the hazard of impalement. On or about August 6, 2012, at the Rainbow Station Pre-School, Marshall Rd, San Antonio TX, employee was exposed to an impalement hazard. There was an uncapped rebar located in the path of travel for the employee to plug in the equipment.
