29 CFR 1910.146(c)(7)(iii): The employer did not document the basis for determining that all hazards in a permit space had been eliminated, through a written certification that contained the date, the location of the space, and the signature of the person making the determination: Weld station #19, center area, east end of the workplace, tank vessel: On 06/07/2012, employees working inside the tank vessel permit required confined space were exposed to atmospheric hazards while engaged in tank surface interior buffing activites and not provided with written certification that all permit space hazards had been eliminated.

29 CFR 1910.95(l)(1): The employer did not make available to affected employees or their representatives copies of 29 CFR 1910.95 and did not post a copy in the workplace: Throughout the worklplace, occupational noise exposure standard poster: On 06/07/12, a copy of the occupational noise exposure standard was not posted in the workplace for review by employees. These employees were engaged in tank surface arc gouging, buffing, and grinding activites with exposure to high levels of ambient noise.
