29 CFR 1926.502(b)(13): When guardrail systems were used around holes which were used as access (such as ladderways), they were not provided with a gate, or were not so offset that a person could not walk directly into the hole: a. On or about August 7, 2012, 2nd Floor of Building, An extension ladder was set-up directly behind a guardrail for a floor hole which allows subcontractor employees to walk to the floor hole and exposes subcontractor employees to falls greater than 6 feet in height.
