Condition noted on or about 08/04/2012.

29 CFR 1926.451(b)(1):     Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units, and the space between the platform and the uprights:     a-Jefferson Ave & Broadway, Westwood NJ: An employee working from a scaffold platform which consisted of 1 & 2 planks as full planking was not provided. The employee was exposed to fall hazards of approximately 24ft to the concrete below. Condition noted on or about 08/04/2012.     Beno Stucco Systems was previously cited for repeat violations of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 451 (b)(1), which was contained in OSHA Inspection Number 313983371 and 314676727.  In regards to Inspection Number 313983371, Citation #2, Item # 1, was issued on 1/04/2011 to a workplace located at 76 South Bridge Street, Somerville NJ 08876. This citation became a final order on 01/31/2011. In regards to Inspection Number 314676727,Citation # 02, Item # 01, was issued on  08/16/2011 to a work place located at 58 Essex Street, Lodi NJ 07644. This citation became a final order on 09/12/2011.

Condition noted on or about 08/04/2012.    Beno Stucco Systems was previously cited for repeat violations of this Occupational Safety and Health standard or its equivalent standard, 29 CFR  451 (g)(1), which was contained in OSHA Inspection Number 311055990 and 313983371.  In regards to Inspection Number 311055990, Citation # 2, Item # 2, was issued on 3/09/2012 to a workplace located at 1116 Mc Bride Avenue, Paterson NJ 07424. This citation became a final order on 04/16/2009. In regards to Inspection Number 313983371, Citation # 2, Item # 4, was issued on 1/04/2011 to a workplace located at 76 South Bridge Street, Somerville NJ 08876. This citation became a final order on 01/31/2009.
