29 CFR 1926.403(b)(1): Examination. The employer shall ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment shall be determined on the basis of the following considerations: (a). Worksite located at 6901 NW 83rd Street, Kansas Ciity, Missiouri- Employees were exposed to possible shock or electrocution hazards in that the extension cord being used by the employees had its' outer insulation breached and had inner wires exposed.

29 CFR 1926.404(f)(6): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent and continuous. (a). Worksite located at 6901 NW 83rd Street, Kansas City, Missouri- Employees were exposed to a shock or electrocution hazard in that the extension cord being used by the employees had its' ground pin removed, thus the path to ground was not permanent and continuous.

Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. (a). Worksite located at 6901 NW 83rd Street, Kansas City, Missouri- Employees were exposed to fall hazard while working on the roof and were not protected from falling by the use of any means of fall protection.
