29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure: a) Employees working on a 2-story residential construction site, approximately 25' above the ground, without adequate fall protection. The employees were wearing PFAS (harnesses), but were not tied/secured to the lanyards as they accessed the roof. The pitch of the roof was 8/12. The employees were preparing to install roof shingles. This exposed them to fall hazards. An employee fell approximately 25-30' off the roof, and sustained multiple injuries. The violation was observed on or about May 21, 2012.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. a) The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and regulations applicable to fall hazards and roofing work. Employees were working on a 2-story residential construction site, approximately 25' above the ground, without adequate fall protection. The employees were on the roof to install shingles and were not tied/secured to the lanyards. This exposed them to fall hazards. An employee fell from the roof and sustained mulltple injuries and fractures. The violation was observed on or about May 21, 2012. ABATEMENT DOCUMENTATION REQUIRED
