Employees who used the tools plugged into this box were exposed to hazards of electric shock or electrocution.

(a)  On or about 8/3/12, at the front of the building, a ladder was not provided to access the planks of the scaffold.  The employees were exposed to fall hazards of more than 10 feet to the ground below.
