29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    On or about August 7, 2012, employees who were conducting renovation work on the copper clad roof of a business establishment were exposed to fall hazards at heights greater than 10 feet to the ground below.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface, and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder.     On or about August 7, 2012, employees who were conducting renovation work on the copper clad roof of a business establishment were exposed to fall hazards.  A ladder used to ascend to the roof and to descend to the landing did not extend 3 feet above the roof upper landing - in lieu of being secured at the top.  Employees were exposed to fall hazards at heights greater than 10 feet to the ground below.
