Location: In warehouse/print shop, 2nd floor of building. On or about 08/08/12; a) Employees were using a hair dryer and a heat press machine that were missing a ground pin.

29 CFR 1910.23(b)(1)(i): Rail, roller, picket fence, half door, or equivalent barrier. Where there is exposure below to falling materials, a removable toe board or the equivalent shall also be provided. When the opening is not in use for handling materials, the guard shall be kept in position regardless of a door on the opening. In addition, a grab handle shall be provided on each side of the opening with its center approximately 4 feet above floor level and of standard strength and mounting. Location: In warehouse/print shop, 2nd floor of building. On or about 08/08/12; a) Employees were exposed to a fall of approximately 12 ft while standing at the edge of an unguarded 2nd floor loading area wall opening to receive shipment boxes. REPEAT STATEMENT OMNI CREATIVE SOLUTIONS INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD, ITS EQUIVALENT, OR ESSENTIALLY SIMILAR STANDARD, 1910.23(b)(1)(i) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 313138604 CITATION NUMBER 1, ITEM NUMBER 2, ISSUED ON 10/14/09.
