West side of building - an employee was exposed to a 6'6" fall hazard, in that, the deck was not fully planked. An employee was required to work from a single plank platform. Violation observed on or about 08/08/12.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. a. West side of building - employees were exposed to a 14'-8" fall hazard, in that, a ladder was not provided. Violation observed on or about 08/10/2012.

West side of building - Employees were exposed to a 14'-8" fall hazard, in that, an A-frame step ladder and a scaffold horse frame were used on a scaffold platform to increase the working height as employees installed the architectural foam band.
