29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were not visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. a) The employer does not ensure that flexible cord sets are inspected for damage and, if damaged, removed from service. This violation occurred on or about August 9, 2012, on the third floor, where employees were using a hand held drill connected to a flexible cord set that showed the visual effects of internal damage from use under an overcurrent condition and cuts to the internal and external insulation. b) The employer does not ensure that flexible cord sets are inspected for damage and, if damaged, removed from service.
