29 CFR 1926.405(a)(2)(ii)(B): Temporary wiring branch circuits did not originate in a power outlet or panel board:    On or about August 7, 2012, the employer failed to ensure that extension cords used to power to a Ridgid shop vacuum originated from a power outlet or panel board. Two 100 ft. extension cords were daisy chained and used to power the Ridgid shop vacuum.
