29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. a) Jobsite -1447 North 17th Street - Employees performing siding operations from the tubular welded frame scaffold were exposed to fall hazards of approximately 13 feet without adequate means of fall protection. Observed on or about 07/27/12. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement and states employees and their representatives were informed of this abatement.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows: a) Jobsite - The 2nd working level of the tubular welded frame scaffold was not fully planked between the front and rear uprights. The working levels contained 1 and 2 planks where two employees were performing siding operations. Observed on or about 07/27/2012. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement and states employees and their representatives were informed of this abatement.

29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable: a) Jobsite-1414 North 17th Street - The employer did not provide each employee performing siding operations from a tubular welded frame scaffold with training to recognize the hazards and to understand the procedures to control the hazards associated with operations, on or about 07/27/12. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement and states employees and their representatives were informed of this abatement.
