29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites:    On or about 8/9/12    Location: 156 Central Avenue, Hartsdale, NY 10530    a) The Mikita right angle block cutter and yellow extension cord were not protected by a ground fault circuit interrupter (GFCI).    b) The red and white extension cord to the Win-Lock hot knife was not protected by a ground fault circuit interrupter (GFCI).

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used:    On or about 8/9/12    Location: 156 Central Avenue, Hartsdale, NY 10530    a) The red and white extension cord to the Win-Lock hot knife was damaged and worn. The cord was bent back over its self for two inches and wrapped in red tape and the outer insulation was damaged/cut.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access:    On or about 8/9/12    Location: 156 Central Avenue, Hartsdale, NY 10530    a) Employees climed on the end frames of a two frame high, eight frame long and a 2 frame high 3 frame long tubular welded frame scaffolds to access the working levels. The end frames were not of the type designed to be used as a access ladder.
