29 CFR 1926.451(g)(1)(iii): Each employee on a crawling board (chicken ladder) was not protected by a personal fall arrest system, a guardrail system (with minimum 200 pound toprail capacity), or by a three-fourth inch (1.9 cm) diameter grabline or equivalent handhold securely fastened beside each crawling board; An employee was working off of a chicken ladder to tear off the roof. The employee was not protected by a fall arrest system while working. The employee was exposed to a fall hazard while working at the peak of the roof. The employee was exposed to a fall hazard of approximately 16.1 feet. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

Employees were accessing a roof and the ladder was not extended three feet above the landing surface. Employees were exposed to a fall hazard of approximately 16.1 feet. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
