(b) Citation 1 Item 2 - 29 CFR 1910.1200(e)(1) - Date by which violation must be abated: 02/22/2013.    (c) Citation 1 Item 4 - 29 CFR 1910.1200(h)(1) - Date by which violation must be abated: 01/18/2013.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.20(b)(2):     The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s)    On 8/9/12, the construction site at 652 Hyacinth Lane in Willowbrook, IL, was not inspected by a competent person prior to the use of a combustible mastic remover.      Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.1200(g)(8):     The employer did not maintain copies of the required material safety data sheets for each hazardous chemical in the workplace:      On August 9, 2012, employees working with hazardous chemicals such as but not limited to No Odor Mastic Remover and the employer did not have a Material Safety Data Sheet.    Certfication is NOT required of this item in accordance with the requirments of 29 CFR 1903.19(c).

29 CFR 1910.1200(h)(1):     Employers did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees had not previously been trained about was introduced into their work area and chemical-specific information was not always available through labels and material safety data sheets    On August 9, 2012, employees using a combustible mastic remover had not been provided with effective training on the hazards of the chemical.    Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
