29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms shall be kept clean and orderly and in a sanitary condition. a) On or about August 10, 2012 workplace was not maintained in a clean conditions due to evidence of bacterial growth such as but not limited to mold growing throughout the entire building.

29 CFR 1910.141(a)(5): Vermin control. Every enclosed workplace shall be so constructed, equipped, and maintained, so far as reasonably practicable, as to prevent the entrance or harborage of rodents, insects, and other vermin. A continuing and effective extermination program shall be instituted where their presence is detected. On or about August 10, 2012 the employer had not implemented extermination program to prevent the harborage of insects such as but not limited to spiders and roaches.
