29 CFR 1926.416(a)(1):     Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:    On or about August 7, 2012, an employee was not protected from an electrical shock while working on and near an overhead distribution line, exposing the employee to the hazards associated with contacting a 7.2kV distribution line.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.100(c):     Helmets for the head protection of employees exposed to high voltage electrical shocks and burns did not meet the specifications contained in American National Standard Institute, Z89.2-1971, Safety Requirements for Industrial Helmets.    On August 7, 2012, an employee was not protected by a hard hat that meet the specifications contained in American National Standard Institute, Z89.2-1971,exposing the employee to an electrical shock as he worked on an energized 7.2kV distribution line.    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.501(b)(1):     Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    On August 7, 2012, an employee was not protected from a fall while walking/ working from the roof of a commercial building, exposing the employee to a fall hazard of 3 stories.      Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
