29 CFR 1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a)  At Keepers Station FEM Assembly:  On or about August 9, 2012 and at times prior; the employer failed to provide  guards  to prevent employees from getting their hands caught in machinery as it moves down the conveyor to the next work station stop point.    b) At HVAC Line:  On or about August 9, 2012 and at times prior; employees are exposed to caught-inhazards from moving equipment on the conveyor located under the employees workstation.

29 CFR 1910.1030(f)(1)(i):  The employer did not make available the hepatitis B vaccine and vaccination series to all employees who have occupational exposure:    a) At Halla Climate Systems:  On or about August 8, 2012 the employer failed to make available the Hepatitis B vaccine to employees potentially exposed to bloodborne pathogens from assisting another employee.
