29 CFR 1926.501(b)(13):  Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b):    a)  At 1752 Benson St Prattville, Alabama:  On or about August 8, 2012 and at times prior; the employer exposed his employee to fall hazards of 20.4 feet to the ground below while installing shingles on a high pitch roof without fall protection.    b)  At 1752 Benson St Prattville, Alabama:  On or about August 8, 2012 and at times prior; the employer exposed his employee to fall hazards of 11.4 feet to the ground below while installing shingles on a high pitch roof without fall protection.

29 CFR 1926.1053(b)(1):  Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a)  At West Side of House on 1752 Benson St Prattville, Alabama:  On or about August 8, 2012 and at times prior; the employer failed to ensure a ladder used by employees to gain access to the roof was extended three feet above the landing area to prevent employees from falls of 20.4 feet to the ground below.    b)  At South Side of House on 1752 Benson St Prattville, Alabama:  On or about August 8, 2012 and at times prior; the employer failed to ensure a ladder used by employees to gain access to the roof was extended three feet above the landing area to prevent employees from falls of 11.4 feet to the ground below.    c)  At North Side of House on 1752 Benson St Prattville, Alabama:  On or about August 8, 2012 and at times prior; the employer failed to ensure a ladder used by employees to gain access to the roof was extended three feet above the landing area to prevent employees from falls of 11.4 feet to the ground below.    d)  At East Side of House on 1752 Benson St Prattville, Alabama:  On or about August 8, 2012 and at times prior; the employer failed to ensure a ladder used by employees to gain access to the roof was extended three feet above the landing area to prevent employees from falls of 20.4 feet to the ground below.
